If you missed our 3 registration dates you can still register for the upcoming season by downloading a copy of our registration form from the link below. Please complete it fully and mail it along with your payment (cheques made payable to CDMHA) to:

Please note that if the Registration is postmarked after Apr. 30th, 2009 it must include the late fee of $50.00. Also, if you are a new registrant with Caledonia Minor Hockey you must include a photocopy of the registrant's birth certificate for proof of age. Failure to follow these instructions means the registration will not be accepted until it is rectified. As always CDMHA reserves the right to limit registration pending roster room on the teams and you may be placed on a waiting list.
